Halvblodet Keoma (Franco Nero) återvänder till sin hemstad efter tjänstgöring i inbördeskriget. Staden styrs nu av Caldwell (Donald O'Brien) och dennes gäng. Även Keomas tre halvbröder har övergått till Caldwells sida. Keoma är inte välkommen och slår sig samman med en av sin fars bekanta för att slå tillbaka Caldwells makt.
