På ena sidan står McMurphy, på den andra den vältaliga sköterskan Ratched (Louise Fletcher), en av filmhistoriens kallaste och mest elaka människor. Varje patients öde står på spel i deras duell.

His frustration is magnified by both his disapproval of Mozart's vulgar behaviour and by the fact that it is he, more so than anyone else, who can appreciate the true beauty of the young composer's work. Although spoilt for choice when the men show up, Andula meets a pianist at a dance and spends the night with him. After he leaves she becomes infatuated and follows him to Prague.
