I Texas många år före händelserna i "Motorsågsmassakern" döms den ökända familjen Sawyers yngste son till rättspsykiatrisk vård på ett mentalsjukhus efter att Texas Rangers-mannen Hal Hartmans dotter har dött under mystiska omständigheter. Tio år senare kidnappar den nu tonårige Sawyer en ung sköterska och rymmer tillsammans med tre andra psykfall.
