Farbror Boonmee lider av en akut njursjukdom och har bestämt sig för att tillbringa sina sista dagar omgiven av sina nära och kära. Till hans stora förvåning dyker avlidna frun upp i form av ett spöke för att ta hand om honom. Även en sedan länge förlorad son dyker upp om än i ny skepnad. Boonmee funderar över sitt öde medan han vandrar genom djungeln med sin familj för att slutligen hamna vid en mystisk grotta på toppen av ett berg som visar sig vara platsen där han en gång föddes...till sitt första liv.
